The School Committee thanks you for taking advantage of this online registration. To print out and mail in the form, choose the printable .PDF document. To e-mail your completed form, choose the MS Word .DOC file.

With either method, you will still need to print and complete the medical waiver/sponsorship form and either mail or bring it to Summer School. The School Committee needs to have a printed copy of this form, just in case the need arises for us to seek medical attention for a child whose parent/guardian is not present at the School.

To make payments with PayPal:

1. Send completed registration form to ljbuder@gmail.com. Please note that you will be paying via PayPal in the e-mail or on the form.
2. Refer to the registration form (line 7, Amount Enclosed) to determine amount of payment
3. Insert family name and above amount into the following 'Buy Now' link to send payment.
